Request Care Form

After reviewing the information on our website, first-time clients can begin the intake process by filling out our Request Care Form. On the form you will specify the counseling type and provide us the contact information of any additional participants for counseling. This form can only be filled out by the primary individual seeking counseling or by the parent/legal guardian requesting counseling for a minor child. Also, all additional adults whose information is listed on the form will be contacted to ensure they consent to beginning the intake process to receiving intake documentation.

Our Intake Process

Once the Request Care Form is submitted,  we can send you (and any other individuals) our intake paperwork. After these are completed, our Executive Director will review your paperwork to determine the best fit for you based on your needs and our counselors’ experience. After you are staffed to a particular counselor, our offices will contact you to schedule  your first-time appointment. During your initial session, you and your counselor will work together to determine goals for counseling, and to make sure the counselor and you are a good fit to address your current situation.

Suggested Donations

All of the counselors at Bridgehaven work on a donation basis & neither Bridgehaven nor any of our counselors are currently paneled with any insurance provider.  Our suggested average donation is $75-$125 per 50-minute session, with a minimum suggested donation of $50 per 50-minute session. If you feel that our suggested donation puts undue burden on you and your family, we encourage you to seek out financial assistance from support systems in your life such as family members, friends, or your local church body. Additionally, when services involving minors are being sought, only individuals who have the full legal authority to consent to counseling on behalf of the minor child or children can complete the intake process. In joint-custody situations where decision-making authority is shared, all parents and/or legal guardians who share decision-making authority related to the minor(s) involved must provide written consent before any services can be scheduled. If you do not have the legal right and/or authority to consent to counseling on behalf of the minor child(ren) without first obtaining the consent of another parent and/or legal guardian, please obtain such consent before beginning the intake process.

Disclaimer

We work very hard to protect the confidentiality of everyone we serve. As such, we are unable to begin the intake process for any adult at anyone’s request but their own. For counseling involving other adults (i.e. marital or family counseling), please inform them of your request before submitting any request for counseling with us. Additionally, please inform them that they will be contacted by Bridgehaven Counseling Associates to ensure they consent to receiving intake documentation as a part of the intake process.
